First enter your name, email address and username. Then you need to choose:

Subject: Request to delete account
Message:
Dear customer support of FanDuel,
I want to close my account on your website. Can you do what needs to be done for that?
Kind regards,
[Your name]

In my case, I received a reply from the support team 3 hours later, confirming me the removal and deactivation of my profile.
